What are forms 1095-B, 1095-C & 1095-A?

Form 1095-B is sent to employees of businesses with fewer than 50 full-time employees. This form explains your healthcare coverage for the year and is for your records only and does not need to be reported on your tax return.

Form 1095-C is sent to employees of businesses with more than 50 full-time employees. This form explains your healthcare coverage for the year and is for your records only and does not need to be reported on your tax return.

 

Unlike Forms 1095-B & 1095-C, Form 1095-A is only sent to those who purchased health insurance on the government Health Insurance Marketplace. This form must be reported on your tax return in order to calculate your Premium Tax Credit.
